Hazard Identification, Risk Assessment, and Incident Investigation

Our Health, Safety, Environment and Quality Management System (HSEQMS) is the library of policies, procedures, tools and templates we use to manage these issues in our Group. Our HSEQ policy requirements are mandatory for all Group business units, projects, contracts, sites and operations. Our HSEQMS is registered as meeting international standards for occupational health and safety, quality management and environmental management systems (ISO45001:2018, ISO9001:2015 and ISO14001:2015). It incorporates a framework for continuous improvement, and guidelines and templates for our teams to develop HSEQ management plans and identify hazards, make risk assessments and establish controls.

Should an incident occur, our HSEQMS provides relevant procedures and guidance for our people to follow, which sets out how incident investigations are conducted. A number of people across our business are trained as incident investigators. In the case of major or catastrophic incidents, investigations may be carried out by an external investigator. Once the investigator finalises their report, with root causes and corrective actions determined, the actions are documented and allocated to individual team members for action with agreed completion dates. Findings and lessons learned are shared with relevant people and teams – in some cases, they may be shared business-wide.
